Community Based Services for Children (CBS) is dedicated to helping children diagnosed with a Serious Emotional Disturbance (SED) achieve success in their home and various community settings. There are a multitude of services available to best meet the needs of children and their families, and ensure that families are able to reach success with the strengths and resources they have available.
Targeted Case Management (TCM)Targeted Case Management is responsible for coordinating services and identifying resources for SED children and their families. Target Case Managers assist the family and treatment providers in treatment planning, as well as collaborating with individuals and agencies involved in treatment.
Community Psychiatric Support Services (CPST)Community Psychiatric Support Services assist consumers with identifying strategies or treatment options associated with the individual's mental illness. CPST is a face-to-face intervention aimed at minimizing the negative effects of mental illness, emotional disturbances or associated environmental stressors which interfere with daily living.
Psychosocial RehabilitationServices are available to assist the consumer in the development and utilization of social and interpersonal skills. It is available to assist the child in improving relationships, social networks, coping skills, and improving overall functioning.
Attendant CareAttendant Care is available to consumers who would otherwise be placed in a more restrictive setting. Attendant Care assists by providing one-on-one support, supervision and/or cuing so that the consumer performs the task by him or her self.
Parent Support ServicesServices are available to assist families of SED children in obtaining resources, training, and support.
Independent LivingIndependent Living and Skills Building is available to assist consumers transitioning into adulthood in developing self-help skills, employment skills, housing, education, etc.
Wraparound FacilitationWraparound Facilitation is available to children on the HCB/SED Waiver to form a Wrap-Around Team, which consists of the child and family, treatment team, appropriate professionals, and community members involved in the child's daily life. The teams determine the most appropriate treatment plan to adequately match the child and family's strengths and needs.
Respite CareServices are available to provide relief from care taking responsibilities for families/caregivers of a youth identified with a Serious Emotional Disturbance.
